It's happened but generally not when staying on points unless the unit type was out of service with none left or you had a problem and they were doing it for compensation. For owners, it would be an unreasonable expectation and even more unreasonable to request they do so.
We reserved a studio at Hilton Head last September for one night (splitting up our drive home) and they upgraded us to a dedicated 2 bedroom!!
We had a march view, the extra room was nice (there was this great niche in the 2nd bedroom that was quiet and dark for our DS -- port a crib), and being able to watch tv in the living room without waking DD & DS was priceless.
It was very unexpected and we thanked the front desk profusely!! What a great way to end our vacation!
Upgrades can happen, but we never ever expect them!! Hey, we are just happy to get a couple of our requests!!

I have to agree with Dean. It wouldn't be fair unless they also charged your account with the additional points needed. There have been occations when (as Dean said) there are not enough of a specific unit because of refurbishments etc. and someone has gotten a surprise upgrade, but it would be VERY unusual and should never be planned on.
We stayed at BWV the night before the 2005 S.S.Member Cruise. Booked a studio and was very surprised to walk in and find a 1BR. DIdn't get to enjoy it much as we arrived about 7:00 pm and left by 8:00 am the next morning. We were not charged the additional points. I assume they were out of studios.
