End of the year School District Special Ed. survey?

Do any of your school districts send their parents an end of the year feedback survey regarding the district's Special Education program/director/services, etc? If so, can you please post a link? Our district is attempting this for the first time. Thanks!

We have district wide surveys, but not specifically for SPED

We have never had one here.

The state of NH has one. The district sends it out to the parents of children in the special needs department. Then the info is compiled and sent to the districts. I just got my survey in the mail yesterday. If you go to your state site you should be able to get one from either the state board of education or PIC (parent information center). The UNH cooperative extension service combines all the information together here. That way it is impartial.
