EMH rides at each park

Just wondering which rides/attractions are open during EMH/MM?

At Disneyland, only Fantasyland and Tomorrowland are open for EMH/MM. I think it's most attractions in both lands, other than Casey Jr and Storybook Canal boats sometimes open late. Over in DCA, it seems like EMH (no MM) is much more extensive, but it might because it's more spread out throughout the park.
