EIC Amendment form

Is there a form specific for this? If not, what form would I use. It's not for me, as I've gotten my income tax and safely put the check away. Last year I didn't cash it until September. Anywho, it is for my sister's 2007 tax. Another question; how far can you go back to make amendments? I thought/hope 2 years.
