Typically, you would get a voucher you exchange at the park for actual AP, which is a good thing since the AP is good 12 months from the time of your first visit not when you bought it. With the current $484 PAP promotion, you have until April 30th to buy the discounted PAP, & Jan 31, 2015 to activate it. If you are just buying the regular $464 DVC AP (not PAP), & traveling before June (potential price increase), I would just wait & get the DVC AP in person since you have to go to theme park Member Services anyways to exchange the voucher for an AP, & might as well hold on to your money until then. The only benefit of buying the DVC AP in advance is to lock in price.

You have to pay a processing fee to have exchange vouchers mailed. The will call option is free and immediately available to link to MDE. Still need to stop at a guest relations location or park ticket window to get the actual passes.

I took advantage of the previous PAP promotion and did what Michael describes above. I opted for Will Call rather than having a voucher mailed. I used the Will Call confirmation number to link my passes to MDE. When we arrived at WDW, we went to the Guest Relations window at the IG at Epcot and handed in our printed receipt for the PAPs and got the actual passes. Very easy!
Yes. You are required to have park admission linked to your MDE profile in order to make FP+ selections in advance. Acceptable forms of park admission that will work are: actual tickets/passes, exchange voucher(s), will call confirmation, or resort package that includes tickets.
I hope this helps. A few months ago I ordered our Annual Passes and was mailed AP vouchers. It has printed on the vouchers that they need to be exchanged for passes when we get to WDW. I called Disney and was told I could input the number on the back of the voucher and link it to MDE. It worked just fine and I was able to link the vouchers to my magic bands and make Fastpass+ ressies.
