Don't wear your pool attire or ratty shorts or clothes with big honking holes in it - or an "offensive" tee shirt (not sure who determines what is offensive). If you're a guy, you may want to rethink the tank top. Otherwise, pretty much anything goes, apparently.

No! V&A is not considered a signature restaurant - that term was invented to distinguish the ones that cost extra on the dining plan. In fact, the OP makes it clear in their thread title that they're on the dining plan, so V&A would probably not be what they needed information about. V&A doesn't even accept the dining plan.
V&A required jackets for men and dressy clothing for women. (Not necessarily dresses - I've been there in a pants suit - and the last time I went most of the men were not wearing ties. But they did wear jackets.)
I forgot about the hats. I remember someone who wouldn't book a signature restaurant because she thought her husband or boyfriend would refuse to take his hat off.
