If you are relatively fit, it's an easy enough walk. We stayed in 08 and walked from there. It takes about 10 or 15 minutes to get into citywalk from the Doubletree and another five minutes or so to get through citywalk. Wish I could have worn a pedometer so I could tell you the exact distance. There is a sidewalk all the way, and traffic lights at every corner, so it's relatively safe also.
I can't say anything about the shuttle. I've read on here that it is crowded, but have no firsthand experience.
I stayed at the Holiday Inn right next to the Doubletree just last week. The walk was only ten minutes and we did it everyday two times a day. I'm not sure about the shuttle for the Doubletree but the one at our hotel was always full and did not leave as early as we wanted. You should call your hotel and ask for the shuttle times.
As the previous posters said it's an easy walk for anyone in decent shape. For someone who isn't in decent shape the shuttle is just putting off the inevitable. You are going to put some miles in no matter what so if you aren't used to walking you will suffer for it.
Walk -- We stayed there last week. It is 1.5 mile walk per hotel lady. The shuttle runs like 4 times a day but didn't leave until 8:25am so we walked. You also have to book it in advance (They actually give you a ticket) b/c it is for 4 different hotels. The Universal conceige (sp?) was very nice, and they are there 730 am to 730 pm. The walk isn't that bad ... about 15 minutes. It is very shady and actually a very nice walk next to the lagoon. We saw many lizards so we started naming them and my 2 yo kept pointing them out. We also drove and parked one day - to me it was still about the same amt of walking That is one BIG garage!

We stayed at the Sheraton four Points on International Drive and it was less than 1.5 miles from the parking lot at the Sheraton to the parking garage at Universal. 1.5 miles from the Doubletree sounds hard to believe.
There is a Google app that lets you put a mark on 1 point, drag it to another point and it tells you how far it is walking distance. I used it once but don't even remember what it's called. You could search for it.
I don't know - she said that was from the front of the hotel to IOA. If you think about it the walk from the garage area to IOA through Citiwalk is a pretty good piece. We never measured it ourselves though - just take my word that it will take a good 15 minutes to walk
You do have a streetlight where you have to cross the crazy busy road, just make sure you don't take your time crossing and you will be ok.
I stayed at Doubletree for 1 night once and made the walk to Citywalk in stilettos. You will be fine. Enjoy your trip!!!
