I have a copy of a Durable Special Power of Attorney for Medical Services. It is a real legal document and has to be noterized. You can PM me if you would like me to email it to you. It's in Word Doc format.
You'll probably want to do a Child Care Power of Attorney granting temporary custody - I found a link to one at http://www.doyourownwill.com/poas.asp, although you can Google that and find many others.
Our Family Lawyer software package had one included and it said specifically that it did NOT have to be notarized. I've done an open-ended one so that DS's grandparents have it whenever needed.

if you google for one, you want to search for child and "permission to treat". That way, you'll get the ones made for children rather than the ones an adult would sign to allow someone else to make medical decisions if they were incapacitated.
