we are taking my baysitter and I need her mom to sign something in case she gets sick. I remember seeing it in a post somewhere but can't find it. TIA!!

You could try over on the Cruise Board. I know I have seen it discussed over there.

I searched all the posts I found but couldn't find anything.

I have a copy of a Durable Special Power of Attorney for Medical Services. It is a real legal document and has to be noterized. You can PM me if you would like me to email it to you. It's in Word Doc format.

Have a great trip!

I had a coworker go through this recently. She just local hospital and had a form faxed over. Also I got mine from Health dept.

You'll probably want to do a Child Care Power of Attorney granting temporary custody - I found a link to one at http://www.doyourownwill.com/poas.asp, although you can Google that and find many others.

Our Family Lawyer software package had one included and it said specifically that it did NOT have to be notarized. I've done an open-ended one so that DS's grandparents have it whenever needed.

Here's a link to one.
if you google for one, you want to search for child and "permission to treat". That way, you'll get the ones made for children rather than the ones an adult would sign to allow someone else to make medical decisions if they were incapacitated.
