we are taking our grandson hes 10 what do we need to fill out inorder to take him? do we need to have the documents notorized?

Make sure you have a good Medical Release form also

I would call and check with Disney on any extra's you need to bring. We brought our neice one time on a different cruise lines and had to have a notarized letter from both parents stating we could take her in and out of the country. A medical release and his medical card would also be handy.
