Docs Question - yes another one :o

Began filling out our docs for an Eastern cruise & have a few questions.

1). Bahamas imigration form asks city where boarded, I would assume Port Canaveral?

2). Bahamas form again, Type of accomodation, which box do I select?

3). Disney docs, Payment Authorization form; Want to use wifes credit card for her KTTW card & use my CC for my KTTW. We know to put down the correct card, but for Following people authorized to charge to my KTTW can we put down each other??? Kids will not have charging privilages.

2) Not sure I know the answer to this one. What are the choices?
3) Only who you put down will have charging privileges. Just put each other on your forms & that's all who'll be able to charge.

2. I believe there is a box to the far right which says something like "cruise/boat" or something like that? I think that is what i put.
3. And when we checked in at the port last week, the CM told us that everyone in our cabin had the same charging priveleges and if we didn't want the children to have charging priveleges then we could go to Guest Services on board to have that deactivated on their cards.
