Do they change and wash bedspreads between guests?

Duvet is likely not washed every cruise. That is the whole point to the two sheets.

I assumed the OP was referring to the covering on the Duvet. I used to have a Duvet I washed the covering every week, but not the stuffing part. I found it to be kind of a pain so I got rid of it. It seems to be what all the hotels use these days. Yeah it may not be washed every cruise. I would hope the decorative blankets are washed people use those. I know my son does if he gets cold at night.

Exactly. In the 32 times I have sailed with DCL, the room and all bedding have been impeccably clean.
 
Do they wash the duvets? Also I saw somewhere that they sometimes change out the kids bedding to be Disney character themed bedding?
On our first cruise earlier on this year they changed the kids sheets out to Marvel sheets as a surprise. We didn’t expect it, they just did it. Not sure if it’s normal as I think the boat was less than 50% capacity.
 
I would think that bed bugs would spread if they didn't wash all of the bedding. Bed bugs on a ship could be disastrous. Way worse than even a hotel.

Then the process would be to put all the used linen in one large bag, take it to the laundry, take the duvets out and then take them back to the staterooms without washing. Why take them out of the stateroom then?

They almost certainly wash everything. They were emphatic about this kind of thing after the restart. Nobody seems to have any real evidence for claiming otherwise.

Bed bugs don't live in the bedding. They live in the mattresses.
