Do I need my ex's signature on anything for cruise?

Yes, I have another question (they are starting to pop up now that things are falling into place). I applied for my DS15's passport and needed my ex to sign and have notarized a form to send in with the passport application. We have since received my DS's passport so that is taken care of. I'm wondering if there are any Disney cruise forms I need to print out closer to the sail date and have my ex sign so my son (and I) can enjoy the cruise and not worry about any surprises when we check in at the port or if he wants to leave the ship (he will be 16 and wants to do the teen excursion) without us on the teen excursion.
 
As far as Disney is concerned, nothing more is needed, even if you and your son have different surnames. US Customs and Border Control recommends that you have a notarized letter from the other parent stating that they are aware of your trip and are granting permission for the child to leave the country with you. It is not required, but they do state that they can delay you from leaving the country if they have any concerns that you might be taking the child without the other parent's permission.

I traveled with my DD (different surname) and niece (same surname) and was not asked for any documentation for either of them from Disney or US Border Control. I had the minor authorization form filled out by my brother and SIL for my niece and didn't even think about it until after we'd boarded that they didn't ask for it. I also had the notarized letter from my ex for my DD, just in case. Everyone assumed that my niece was my daughter, even though I'd listed her address as different than mine and listed her emergency contact as her parents.
 
Yes I am his mom but we dont have the same surname as I'm remarried. I'll make sure "just in case" to have a notarized letter from his dad before we leave for the cruise. So I should be ok to sign off on my ds's teen only excursion where they leave the boat with CMs of the teen group? I know there is a page that needs to be signed for that.

Is that on Castaway? I can't see a problem with a parent signing off on that. :)


I'd have the notarized permission letter, definitely. Might as well, right?

Although I don't have personal experience with this specific excursion, I don't see where it would be an issue that you are the only one signing permission for him to go on the excursion. I was concerned about whether I needed my brother or SIL to sign permission for my niece to go on the parasailing excursion on CC with us, but it wasn't a problem at all. I had a notarized letter from my SIL granting permission for me to take her off at all ports, take her on any excursions, and seek medical care if need be. I never needed that letter, although I carried it with me any time we got off the ship, including at CC. I was the adult responsible for her for the duration of the trip and as such, DCL accepted my signature as the only one needed to grant permission for her to do the parasailing.
