Disney Dining Pocket Planner

Can anyone tell me how to get to a pocket sized or mini dining plan. I have seen one recently and actually printed it out, it was on one sheet that you folded into a pocket giude. I gave it to my father, now MIL needs one. Any help is appreciated.
