I e-mailed them off the
disney store website. This is what I wrote:
"How do you receive an information packet to activate a Best Guest
account? I have a log-in with Disney.com, however, I am unable to
sign-up for the Best Guest account without an activation #.
Thank you for your time."
The reply I received is:

I knew I didn't qualify for the free gift (my order was just over $100.) but I wanted my total to go against my ordering $ amount total. Not sure what will happen. I'm still not clear on this yet. It would help if I do receive a "package". Maybe it'll explain things better.
