Disabled passes ?

Does anyone have any information on how we go about obtaining a disabled pass, as our youngest son (18 months at time of travel) has Down's Syndrome and I have been told that you can get passes for disabled guests to avoid waiting in the queues.

I appreciate that he will not be able to go on many rides, but all the same I'd like to avoid queing too long with him on the rides he will be able to go on. I presume also we will only be able to use the pass on the rides he can go on ???
