Disabled Access Card Question

I have to admit I am at a point where my knowledge is over.

Can someone tell me how one of the medical certificates from the doctor you need to bring is supposed to look like? A co-worker wants to take his kids, and his daughter would qualify for one of the passes. However, their doctor has never filed one in English. I am myself at a total loss what info is needed too. How detailed does it have to be?

If you'd rather answer by PM if you can help that would be great too. Any tips and hints are appreciated.

its not technically a certificate. i take a later that has all the hospital/drs info on the letterhead (top of sheet) and then full name and address of ds2....

then the letter just details his issues and his 'disability' so - autism, difficulty in queueing, needs familiar things etc....

its just an overview of the problems, so that the cms know what is wrong, and what category they best fit under - and of course to be dated less than 3 months before trip...
