DDP and salads

When your on the DDP and there is a salad (such as LC,CDF), do you also get it. Or is it just, Appetizer, Entree and Dessert, and if it is this way, can we exchange the Appetizer for a salad?

I've read reports where folks have reported that they've gotten a salad as their appetizer.
