I have requested a wheelchair accessible room for an upcoming cruise. I chose an accessible room because I needed a room where I did not have to step over the tub and if seas are rough I could sit down and not worry about loosing my balance. Do I need to send in the guest medical information form?

I have never had to fill out a form when I have stayed at any Disney resorts.
I have limited walking ability and balance issues. I have a form of muscular dystrophy, CMT. I am going to bring my wheelchair because I don't know how rough the water will be with the rocking of the boat, due to the fact it is hurricaine season.

Have any of you not turned in a form?

We turned in forms for the accessible stateroom for my husband and another for me. We had late dinner and as a diabetic I needed the earlier one--

A friend was cruising with an autistic child who had a few medical issues. She also filled out the medical history on her child JUST so the dr on board would be prepared in case of emergency with the treatment the child could need.

The accessible rooms are larger and people would request them without a need if they didn't ask for a doctor note.
