Cruise document? Medical Info Form?

I was just over on the DCL web site previewing the cruise document section that you can now download. Having never cruised DCL, I was wondering about the medical information form. DH & I don't have any real medical issues. How do we fill that out? And does it have to be signed by your doctor? I know you previous cruisers will know!

The medical info form is used if you are requesting anything from DCL for medical reasons, as well as to verify the gestational age of a pregnancy. It does have to be signed by a doctor.

Examples of special services would be a refrigerator (rather than "beverage cooler") in the room to store medications, a handicapped room, a physician or nurse to start an IV for already prescribed medications, etc.

If you have no medical issues, you don't need the medical form.
