I've seen cruise prices as low as $799 adult/$399 child. Is this realistic? How much should I budget in addition to the actual cost of the cruise for extras?

Extras include soda (either the sticker plan or individually), alcoholic drinks, spa treatments, arcade cards, excursions, souveniers, and tips....
Our family of 4 just did a 4 day cruise. Dh and I had a drink or two a day, had the soda plan, bought several souveniers, rented snorkel equipment and floats on CC and tipped well---our stateroom bill was, I believe, just over $600. ($200 of that was tips)
We did not do any formal excursions or spa treatments.

So it was reasonable, IMHO. Compared to our bill after 12 days at the World, it was a STEAL! :)

Hope this gives you an idea of extra costs.

Forgot to mention, you'll also need airfare or gas money to get there, maybe a hotel the night before, and either port parking or shuttle/cab/limo.

You also need taxes, insurance and port fees which can add up.

I believe port charges are included in the cruise fare. Taxes and insurance are not.

........Plus travel insurance and transfers. Myself, DW and DD's 12 and 10 spent 900.00 on the ship for the 4 night Wonder. That included spa treatment for DW my Espn mug habbit (beer) pop mugs for DD's pics at shutters arcade and tips. We just spent and didn't count until the credit card statement show up the following month. We did spend $$$$$ shopping at the ports also. It is definately less expensive than WDW.
