I'd be surprised if anyone has one. It's against policy to receive money for transfers so I would think most do not use one. When I transferred points last year, I did not use one.
I'd just write up a document that states that "I, readytogotodisney, will transfer XXX #of DVC points from the 201X Use Year for XXX DVC Resort, to the membership #XXXXXXXXXXXX owned by XXXX XXXXXX, after confirmed receipt of XXXXX . The transferror will provide the transaction confirmation number after the transfer is complete and the transferree will confirm receipt of the acceptance of the transferred points. "
Phone numbers, addresses or any other information desired can also be included. In most cases, transfers are pretty straightforward and are always irreversible transactions. If the transferee is not comfortable without a contract I see no reason not to provide some sort of simple document outlining what is expected by both sides of the transaction. Contracts for transfers are certainly not the norm however.
