I can't answer you entirely - it seems to be up to the waiter/ which restaurant etc.
Here is what we did: It was only DD, 5 and myself and we ONLY used one kid voucher - no adult.
Rainforest Cafe at AK: Waiter asked us if we wanted to use the voucher for DD or me. I kind of looked at him strange and said "It's a kid voucher - for her" The way he acted was that he was going to let us order any entree off either menu. What we actually ordered:
Caribe Chicken Appetizer - plenty for my meal
Kids Pizza
Strawberry Smoothie Drink
Kid Cookie Sandwich Dessert (were told we could order off the adult menu, but not Volcano, but we were in a hurry so I wanted something small to finish quickly)
Total would have been $22
ESPN Club: My mom was with us so we just used one kid voucher and planned on paying for hers.
Ordered:
Macho Nachos for APP - huge and could not finish
Kid Meal of chicken fingers/fries
Drink (could have gotten Smoothie, etc. off adult menu)
Child dessert (again we were told we could order off the adult menu but I was stuffed!)
Waiter came with the bill and he had put my mom's meal on the voucher instead of the kids meal (Mom paid for the kids meal).
He said the voucher does not say you have to order a kids meal.
Total $27
As you can see it looks like you could at least at some places order off the adult menu for all phases of the meal - probably depends on the server and the restaurant. I love appetizer type foods so it was ideal for me to eat these as my meal. With DD only 5 yrs I couldn't justify ordering an adult meal even if they let us - she doesn't even finish the kid meals. For older children though I would definetly enquire if they could order off the adult menu.
For these two vouchers together we saved $25 over what it would have cost. I am planning on ordering a few more for my next trip, just DS and me.
