complaint letter? I need one.......

Hello, does anyone know of a rough draft online of a good complaint letter? I need to contact a company with one and here are some guidelines:

it's a major company
they have "misplaced" some very personal financial info we provide to them
I'm afraid of where this "lost" info could be now (who has access)
I feel they have been neglectful with my information
We have been calling them almost daily for two weeks - they always say they will "find out and call us back" and they haven't called a single time.

So, I need a complaint letter.

Any advice would be appreciated.
