Children's menu extra charges on DDP

On some of the restaurant's children's menus, it says "$2.00 extra" for certain choices, such as steak. Are these choices included in the DDP, or will we be charged the extra $2.00?

From the "all you ever wanted to know" sticky FAQ thread:

Of course, "add-ons" are never included, nor are items intended for more than a single diner. In some cases two diners may be able to order an item-for-two. Also, both Whispering Canyon and Le Cellier might have surcharges for the more expensive child meals. Ask before you order.

In other words, be prepared to pay out of pocket, but some posters have reported kindly server CMs letting the charges slide.
 
The $2.00 extra for steak in places like LeCellier are not payable by DP participants. They are for people paying OOP. Those items (at least at this point) are included in the DP.

Add-ons are things like shrimp along w/ steak, used to be lobster tails, etc. Those items are extra and I'm not sure at this point if the server can "let it slide" any longer.
 

we were never asked to pay the upcharge on the DDP (July6th-12th) and our kids ordered a couple time with an upcharge, like the $2 chagre for a beef kebob kids menu item.

We never ordered in addition to the menu.
