We have friends who always want to go with but when it comes time to book, there's 100 reasons they can't go. Not a big issue cause we know not to rely on them. Well now 1 of them has booked and put down her $200. When it comes time to make ADR's if I include her with ours what do I do if she backs out later? Will they just change it from 4 to 3 people if I call or will we need to cancel & hope to get new ones?

I would make the ADRs for the maximum number of people that might come. It's easier to downsize the party than it is to increase the number of people. For most ADRs you don't have to change the party size. Just tell the CM at the check-in podium the correct number of people (I've had to go from a 4 to a 3 several times and it's never been a problem).

The only places where I would make an effort to change the party size beforehand are at the pre-pay venues (like CRT, HDDR, etc.).
