Changes to Disney Restaurants over time?

I'm wondering how the new Disney restaurants and menus are developed. Do they ask for public opinion? Do they just look for a perceived hole in their current offerings? I don't know if anyone here has any insider info, but the recent threads about the opening of Sanaa have made me wonder. Why would they decide to change the classics? How many years do most of the restaurants stay around?
What are some of your favorite Disney restaurants or menu items that are no longer available?

They do tons of market research coupled with opinions from the general public - people who have visited the parks, dined then taken the time to write or call Disney about their experiences.

There are a lot of menu items that I miss from Disneyland, but on the other hand they've added so many delicious and healthy (and diverse) menu items I barely miss the old ones. LOL
