Can someone help design?

Last year, an amazing artist made me the MS logo that is in my screen. I am wondering if someone would be willing to make another.

I would like the orange ribbon, any cute character photo and the words.....In sickness and in health, forever....or something like this. I am not creative. I can't tell you how many compliments I got on the first one (that I still wear proudly).

I LOVE it but, can I ask to see it without the side art. The ribbon with the faces in the opening is just grabbing me. Are you able to remove the side stuff but, keep the forever? Thanks so much for taking your time to do this for me
