Can anyone do ID tags for my boys?

I am just wondering if anyone could do ID tags for my two boys to hang on their lanyards in case they get lost. I am just looking for something about the size of a license with some personal information. It can be blank with spaces for me to write the information or I can give anyone who is will ing to give this a shot the information to put on these tags. I would greatly appreciate anyones help.

These are awesome. I will definitely be using these. Do you have one with Cars. This is my 2 year olds favorite right now. I will use one already done for my 15 month old nephew.
