Can a DISigner create me a Lost Card?

I found this wonderful Kids for Kids Walt Disney Book and in the back it has a Lost Card. It's very simple and plan but I was hoping one of the DISigners might try adding some Pixie Dust to it please.

It has to sides (so if you fold it in 1/2 it is the size of a business card -also the top part looks upside down so when you fold it you can read both sides by flipping left to right instead of up and down) Hope all that sounds right.

The top part says:

LOST CARD DIRECTIONS:
1. Stay where you are.
2. Find a Cast Member. (They have on the special name tags.)
3. Give them this card, or tell them you need to call your parents.

The bottom of the card says:

My name is _____________________________
I am staying at _______________________________________
My parent's name and cell phone numbers are:__________________________

If a DISigner is willing to make a few of these I would love to have Tink, Ariel and Woody for my three kids. OR maybe it would be cute to do one with Mickey as a Policeman or showing the characters helping each other.

That would be wonderful! Are you the author of the book (I see it posted in your signature)? It is such a good book. I am reading it myself before I give it to my oldest who is 11.

Can you do the DISign with the information from above? Or do you need something else?

I purchase the laminated business card packs at my local office supply store.

See if this one prints clearly before I attach the others. If it doesn't, I can save it in a different format.

I was able to print the card today at work. Everything looks great. Thanks again for doing these. Would you mind posting the other two with Ariel and Woody?

I love the cards and can't wait to include them with the kids lanyard. I just pray we don't have to use them.
