Do you mean the stuff they do in the clubs? No. You don't have to sign up for any of that. Any child in the club at the time the activity happens can participate. Or no, as they wish.
 
OP, its possible you're referring to some of the "ticketed" events you may have read about. The "Princess Tea" is an event that I believe you can book ahead of schedule. However, the princess meet and greets, the Disney Junior breakfast, and the gingerbread house making (if you're doing a holiday cruise), are all things you register for when you arrive at the ship.

You could also be talking about "registering" for the kids club in general, which can be done in advance.

Your friend is correct. You want to register your kids for the kids clubs. (I don't have kids in the age group but I think it works like this). You fill out some basic information online. Then you can complete the registration process at the cruise terminal or once on board at the actual kids clubs.
