It will be my nephew’s 5th birthday when we go to WDW next year. Do any places in WDW do anything special for guests on their birthday?

Make sure you get your nephew a Birthday button. It might get some pixie dust in terms of free desserts or snacks, but there's no guarantee of that.

Everyone celebrating some milestone. If he gets a special treat that is a wonderful pixie dust surprise. Majority of places don’t do anything because Disney would lose so much money since so many people have a button on.

I would make sure he has on a special Tee shirt proclaiming his B DAY or a B day HAT and a Happy B Day pin if he'll wear one.

It should result in Happy Birthdays from anyone in general and some cast members who are paying attention!
