Can anyone help with Beeper MD. Just booked and at the end they ask for an HSRA form and government ID. What’s an HSRA form. And will driver’s license from Ontario work?

Never got that when I booked for us over Christmas. They do need a piece of ID and driver's licence is sufficient.
