Bahamas - Immigration Form Question?

Hi everyone

We are getting close and I am filling out the Bahamas Immigration Form and I have two questions:
For Dreams Unlimited for Travel Agency, What City do I put?
and
Do I sign my children's name on their forms?

Can't answer #1 for you, but question #2 yes you can sign your childs names.
