Assistance needed on Cruise Documents

Just received our cruise documents yesterday for our April 3rd (4 Night Wonder) Cruise. I have a few questions and need some help on our Bahamas Immigration Form. What do I put for the following:

-Passport Number (if I don't have one.) Do I just leave this blank?
-City where we board the boat (Port Canaveral or Cape Canaveral?) Plus it won't all fit in the boxes. Just write what I can?
-Travel Agency and City of Travel Agency? I booked through Disney Vacation Club
-Intended Stay (nights?)
-Hotel Name/Intended Address

Please help with what you can! Thanks a lot!

I am just filling out my paperwork as well (4/17 Wonder) and I left the passport part blank since I am using a birth certificate. I put PTCANAVERAL as my departing city, I left the travel agency thingy blank and didn't fill out the # of nights since we are just docked there for the day. I think you need to fill that out if you plan on staying at their hotels or something. But I'm a newbie to this, so hopefully someone else can answer our questions.

We are going on the same cruise as you! This will be our first cruise, so mot sure I have these answers correct.

Here is what I did:

-Passport Number (if I don't have one.) I left it blank.

-Travel Agency and City of Travel Agency? I booked through Disney Vacation Club Our TA's address was printed on the first page in our travel booklet which was in our cruise documents.
-Intended Stay (nights?) I put 1
-Hotel Name/Intended Address I put "Disney Wonder"

Guess I need to start looking for my docs as we are on the same cruise. Nothing yet Luckily I already know about the air portion as we received that a while ago since we need handicapped assistance.

On the immigration papers, we had to do a rush fill in at the terminal as we had ??? too a couple weeks ago..... no passport write in birth certificateport canaveral....cape canaveral should be fine
I just wrote name of who we booked our cruise thru... disney .. i don't think I put an address
intended stay # of days you will be in Bahamas
Hotel name... is name of your ship
