You have never been able to upgrade tickets to the DVC AP by phone. I have always had to do it once I arrived at the park. But, they said they should be available before the 50th anniversary starts so I think anyone going in September has a great chance of being able to upgrade when there!

Can you upgrade them in person on the last day of your ticket, or does it have to be before the last day? Historically speaking, that is.

Historically, you have been able to upgrade before close of business on the last day that your existing ticket is valid.

first day you used the pass is my understanding -- but I'm not entirely sure.

It will start the date you first used the Hopper pass. So if July 1, 2021 your AP will expire at midnight July 1, 2022
