Anyone ever find Index Card Templates?

Just wondering trying to put this years ADR's ect on a card. I have a template someone sent me but cant get the card size to enlarge.

I've never found a free one that works but I did see Avery Index card templates for Ink jet printers at Walmart today. It was 5.79 for I think 12 sheets. I'm not sure how many index cards were on each sheet.

I immediately thought they would be perfect for Disney touring plans.

I'm not sure if this is what you looking for but this is what I use. I downloaded them from the 4 shared file and I have been able to customize them for my trips.
