anyone do custom embroidery? Need a birthday present

I want to get a polo shirt embroidered with a birthday cake. Left chest, just like a team logo. We'll be celebrating a birthday at WDW in May and this would be a perfect gift.

Does anyone on this board take small jobs for a fee? If not, can you recommend an online shop that is reasonable for a single item? I've tried a couple of google searches, but everything seems geared for large orders.
