The wheelchair info is pages 43-47; kind of nebulous- it’s ‘mishandled‘ wheelchairs, which includes damaged, misdirected and lost.
I suppose it doesn't really matter if it is damaged, lost or misdirected, the net result is the same of not having the device available to use, so lumping then together makes sense.
 
I just flew American last week (which is not my fav airline but not for ada issues). I sat in coach as it was a last minute flight and a short one. I used an aisle chair and they have brand new ones with an excellent design. It always takes a while to deplane but that is more due to the airport then the airlines. I am a very frequent traveler and except once with Air France in Paris I have not had any issues. I use a lightweight folding manual wheelchair.
