Your personal item MUST fit completely underneath the seat in front of you, not sticking out into the foot space in any way - not even a strap hanging out, at least according to JetBlue. Eighteen inches doesn't sound as if it would fit, but check your specific airline. If the exact underseat measurements aren't provided, call them.

Just my opinion, but it's not reasonable to intentionally carry on luggage that doesn't comply with an airline's maximum, with the intent or expectation or hope that one will be able to be exempt from paying luggage fees if any luggage ultimately has to be checked.
I mean for it to go under the seat. I think it will fit but I don't know the dimensions of the seat, do you. United/Continental do not define the dimensions of "Personal Item", thus why I am asking here.
Personal item refers to purse, laptop, etc...... you are trying to take a second bag to avoid luggage fees. Also, keep in mind that not all the area under the seat in front of you is available for every seat. Some have a equipment box under them that restrict the size of bag you can put under it...
Essentially, the personal item is something that you would never want to be away from, such as valuables.
I personally have taken roll-a-board and a laptop bag with no issues, with the laptop bag fitting fully under the seat in front of me.
