Dumb questions here.

Is an ADR just a way of reserving your table, like you would do any other
restaurant no matter where you were on holiday

Can you book ADR's on line or do you have to phone the restaurant. And if
you have to call, where do you get the contact numbers from

Do you have to request ADR's for breakfast

An ADR is setting a time that you would like to eat. It is not a reservation and you will get the next table that opens up at your time.

You have to call to make them and the number I generally use is 1-407-WDW-DINE.

You give them the restaurant and time that you would like to eat and they will tell you if it is available or what the closest time is. It is almost a necessity to make them for any table service place now. Walking up and getting a table usually doesn't work anymore.
 
You need an ADR for breakfast if you plan to eat at a "sit-down" type of restaurant or a character buffet. If you plan counter service, you don't need (and can't make) ADRs.

An ADR is basically a reservation. You are given a time and are asked to show up 10 minutes before that time. You will be seated within 15 minutes of your time. It differs from a standard reservation in that you will be seated at the next available table for your party size. They do not have a table with a little sign on it saved under your name. If you do not show by 15 minutes after your scheduled time, your spot can be given to someone else. If you show up later than that, they will make an effort to work you in, but no promises.
