It's not that serious.
People's pins direct the booking CMs to the free dining pkg code. There are tons of new CMs coming in constantly. It's very easy for them to apply it on accident thinking its a broad offer and not understanding. Once it's done Disney won't take it away. Most know better, but it happens.
When they look you up or your reservation it will show them if you have a pin code so it's also entirely possible they don't ask you for the pin. They already see it.
It's nothing to count on, but it certainly happens and it won't be done on purpose as the CM will be reprimanded.
This makes the most sense out of everything I have read regarding the 'Giving' of pins.
