Writing reviews for yourself?

Are there any good websites that help with wording a review?

I have to type up a list of things I have accomplished and what I need to work on. I seems to start each point with:

Keep updated reports
Keep updated log of outsanding issues.

I want to make it sound better as I am not a very good writer.
 
I would suggest using complete sentences instead of short phrases. Focus on action verbs and detail your accomplishments. For example:
During the review period, I accurately logged ### outstanding issues in the department issues database.
Each week I produced accurate and timely reports on our unit's production vs. our goals.
I successfully resolved ## customer problem calls with a ##% satisfaction rating.

It's important in your own review to clearly specify accomplishments so you blow your own horn. Often managers will not recall everything you've done and this is your opportunity to remind them.

For development needs, try to phrase those in a positive way. For example, suppose you need to work on customer service skills. You might write:
During the next review period, I will take a class on effective listening skills to improve my service to our customers.

While that identifies a development need (listening skills), it does so in a positive manner. HTH
