Which forms need to be completed?

I recently printed forms from the DCL site, i.e., Guest Medical Information, Payment Authorization, Embarkation-Depature Form. Are the Guest Medical Information forms mandatory if you are a healthy person with no medical conditions? Also, do you have to complete Embarkation forms for your kids also, or just one per family? Thanks for the information - this is our first cruise.

No need to complete the medical unless you have a medical condition.

they will also send you a ticket book with the forms you need to fill in.

the payment form is one per cabin
the customs form is one per person, kids too.

medical forms are not necessary unless you need to notify them of special conditions,requirements
