Considering my first trip was as a child in 1977, I can guarantee we ate at a counter service place in the MK. Other than that, I think the only thing that stands out for us is going to the Hoop Dee Do Revue. :goodvibes

Ok here goes:

MK:
Counter service - 1977

Epcot:
Lotus Blossom Cafe - 1987 (I remember this because my friend and I wanted to go to the Odyssey, but by that time, the place had been permanently shut down.)

Disney/MGM(as it was back then!)
Sci - Fi Dine In - 1992

AK:
I want to say Tusker House - 1998

Downtown Disney:
Fireworks Factory - 1992 (Gosh I miss that place!)

Resort:
Does the Hoop Dee Do count? It's technically at Ft. Wilderness. ;)
The only other places I remember going were the "buffeteria" where Chef Mickey's is now and the Papeete Bay Verandah for Tonga Toast!
Oh, both of those in 1987.

I don't remember doing a character breakfast until I dined in the castle with the princesses on Walt's 100th birthday in 2001. :)

Heck, I remember when the Crystal Palace was a really good "buffeteria". I miss that concept. And the "handwich". Bring those back. :)
