Yes when the park opens to regular guest. Not during MM. And assuming the attraction is operating, if the attraction opening is delayed FP will not be available.

Correct. FPs are NOT available during MM. Only during regular park hours. (but if you're there for MM, you can be in front of the machine at ST & SM BEFORE the great unwashed non MM hordes arrive. )

Ski makes a good point. This has happened to me a couple of times.

Went to Indy to pick up my second FP at 8:45am....the ride & machine was down. Didn't miss a trick, kept right on going out to Splash & picked up a FP there & was at my next originally planned stop (at HM) only losing a minute or two.

Even if you make a plan...be prepared to go to a plan B or C.

Not necessarily - FPs may not be offered for certain rides on less busy days. I've seen this most often with BTMRR.
