Nothing as far as I know, just different terms used by different hotels/people.

Concierge level and Club level are the same. However, the Guest Services desk in the lobby of hotels has also been renamed Concierge Desk. That's where you can pick up copies of your Dining Reservations, have Boarding Passes printed out, ask questions about WDW, etc.

Disney now refers the former "concierge level" of rooms as Club level. They offer Concierge services and provides amenities for guests staying in a room offering on that level.

The Lobby Concierge is just a newer name for Guest Services at the front desk of the hotels.
