What's included in Kids QS?

I read through a few pages of the sticky and tried searching but couldn't find it.

If we're ordering at a place like Mara using QSDP, where the kid's meal includes juice or milk, do they get to pick another drink? Or can they pick something else beside milk or juice?

And do they also get to pick a cupcake as dessert or are the grapes (part of the kids meal) considered dessert?

Usually you get what is included in the kids meal and no add'l drink or dessert. However, in several places the cast members just let us order off the regular meal, even when there was a kids meal option. That seems to at the discretion of the cast member.
