Mine would be RnRC - it's not the ride itself which I don't find too bad, it's just the wait at the start, knowing that it's about to launch. It makes me anxious for too long afterwards!
I was 18 years old when I rode Space Mountain for my first and last time. It was too much for me not knowing where the next turn or drop was. It messed me up pretty bad.
I also tried Mission Space for the first time on our last trip. I am a little claustrophobic and I'm a big guy so I was held in the seat pretty snug. I was getting close to a panic attack by the end of the ride. I couldn't bolt out of that seat fast enough.
Captain EO is pretty much it for me. That's 15 minutes of my life I want back still and it's been over a year. Astro Orbiter scared the daylights out of me but I think I've convinced myself to try it again this year.
The Nemo ride in Epcot. I totally missed the point of that one, bored me out of my mind. I'm also not a fan of the teacups. Only needed to throw up on that one once as a child to know not to do it again! And the barf covers they put over the cups that are "contaminated" gross me out to. Just the thought of how many people hurl on that ride on a daily basis to necessitate those covers!!!
My immediate response was "The Hall of Presidents" but after thinking about it I'll probably be back after the elections to see the new animatronic of whoever the next President is. Now, the Tiki Room I have no reason to go back to. I wouldn't even waste my time showing it to someone who has never been to the park.
Dinosaur more for the noise and jarring. Astro was just to high and spinny. I do fine on coasters though. I think because it coasters are moving not just going around in a circle. Can't go on ferris wheels either. Dumbo and Magic Carpet ride are borderline
Astro Orbiter - Not worth the wait. We only did it because my DS (6) really wanted to do it. The wait time was listed as 20min, so I said ok... it ended up being 45min.
Stitch Great Escape or whatever it's called. I always promise myself not to do it again. But if there's no line and it's a walk-on, I foolishly try it again. Rude - Boring!!! Sorry...
any motion simulator ride. I just can't take them. The sole exception (so far) is Soarin'. I'm not sure why. Maybe because it's really open and not all herky-jerky but Star Tours gives me a horrible headache. I don't know about Mission Space, not willing to try, but Body Wars almost made me ill.
The other, and I'm almost crying to type this....The Tea Cups. I always loved the ride when I was little and now I can't even look at it. *sigh*

Not a fan of Star Tours- motion sickness (hope it's better now) or Dinosaur although I will probably try that one again this year- it's been a few years.
