What form do I need so that I can get my DN12 medical attention while we are in Florida? Temporary guardianship? Or just a medical release?

My son and DIL just give me a medical release for my granddaughter. It states that I have full authority over all of her medical decisions without further consultation from them. This way if a decision needs to be made quickly, I can do it.

Make sure the parents sign it and have it notarized.
 
Probably a limited power of attorney signed by both her parents. I'd call a hospital in FL and ask them what documents they would require in order to treat her.
 
I have traveled several times with nieces and nephews. A handwritten note is fine, but get it notarized. If it's not notarized there are limitations to what medical treatments you can authorize. SInce I usually travel by air, I make sure it inlcudes the dates we are traveling and where we are going.
